In your opinion, have you have been involved in a medical situation that you consider to be negligent or sub standard care?
We are looking for reports of cases along the lines of the Insight Films Homepage as an example, and would like to receive a report synopsis either by sending an email to the email address at the foot of the page, or download this printable PDF form which can be filled in and mailed to our P.O.Box. Details received may be used in a forthcoming documentary film which will be distributed on the internet worldwide. Please include your full contact details.
An alternative you may like to use is our on line email form - where again we would remind you to please fulfill our four requirements for any case synopsis.
We ask that the following points be addressed for a case synopsis -
1) - You believe the case involves negligence.
2) - You believe the case involves sub standard care
3) - You believe damages were received.
4) - You are prepared to appear on film.
5) - You must be prepared to make medical records available to verify your claim.
